13568 facilities in 1739 cities across
five states whose licensing agencies publish their records openly. Every
row shows the state’s own license, capacity, inspection, complaint and
citation fields exactly as recorded, with the fetch date
(2026-09-06). Where a state’s record lacks a field, the page says
so.
This site is not a ratings site. It
does not score, rank or recommend facilities, and it never fills a gap in a
state’s record with a number from somewhere else. It shows you the
public record and links you to the agency that keeps it.
